Advance care planning is a process that allows people to make decisions about their future medical care. It provides direction when a person is unable to talk about their own health care choices. Advance care planning includes advance directive documents as well as on-going conversations with loved-ones and decision-makers.
Five Wishes is written in everyday language, making it easy to understand and complete, while covering personal, spiritual, medical and legal wishes all in one document. It allows families and caregivers to know exactly what you want, and it’s also legal in nearly every state. The Five Wishes Program for Healthcare offers a robust suite of tools and resources providing a solid foundation for meaningful communication.
Also known as a living will, an advance directive is a legal document that lets your healthcare team know your preferences for the medical care you would want to receive in the future. This is different from a financial will that typically names someone to manage your estate or provides for the transfer of your property at death. You do not need a lawyer to complete your advance directive – all that is required for it to be legal is your signature and the signatures of two witnesses.
The Conversation Project is a public engagement initiative of the Institute for Healthcare Improvement (IHI). Our goal is both simple and transformative: to help everyone talk about their wishes for care through the end of life, so those wishes can be understood and respected.
Council funding is, at times, used to develop publications that serve as resources for persons living with a developmental disability, their family members, and support teams and that support the accomplishment of the goals and objectives of the Five Year Plan they were funded under. In addition, the Council provides grants for educational activities of community partners.
Choices are individual and an implied human right for all citizens — yet many people with disabilities are often not able to fully experience a self-directed life. This series considers supported decision-making, the dignity of risk, guardianship, and additional options for protection without interfering with a person’s autonomy. This 5-part vodcast (video) and podcast (audio) series provide an overview of what to expect from the Self-Determination series, what Supported Decision Making means; the continuum of supports from least to most restrictive, what you need to know about Guardianship, and why the dignity of risk matters.
