Developmental Pathways (DP) is hosting a public forum to inform the community about upcoming system changes, answer questions, and address concerns.
This town hall will focus on:
CFC (Community First Choice). A new program within the state plan that has functional eligibility element. Specific waiver services will be accessed through CFC. To learn more, please visit, https://www.community.dpcolo.org/community-first-choice
SIS (Supports Intensity Scale). This is a tool used to determined support levels for various waiver services. The SIS will be decommissioned on July 1, 2025. An interim tool will be available until the new tool is finalized. https://hcpf.colorado.gov/new-assessment-and-person-centered-support-plan
