Journey of Care is contracted with CO Medicaid to provide no-cost remote/virtual supports. This is a fully covered, underutilized benefit. We offer a no-cost tablet to connect with our trained care team, as well as any safety equipment someone might need to stay independent.
Quantity:
The monitoring equipment could be fall detection, blood pressure, stove sensors, wander guards, etc and everyone get emergency buttons. This is a fully covered benefit and no cost to the member under HCBS waivers and Community First Choice. The services are not diagnosis based, but rather needs based and approved by the Case Management Agency.
